I would not go so far as to say medical care here would be equal to what is available in the great teaching hospital in the US however there are some similarities in other areas.
If you can afford a hospital such as Clinica Biblica or Cima Hospital you will receive care that is on par with US medical care. They have well trained doctors, modern equipment and excellent support facilities such as labs. The cost is a fraction of what would be normal in the US. I had a colonoscopy done at CIMA Hospital. The cost for everything hospital, doctor, anesthesiologist, lab work was $425.00. The same procedure in Miami would run about $4000.00.
On the other side of the coin if you are a Tico or a gringo without money you would use the Costa Rica Social Security health care (CAJA) This can run all the way from outstanding to horrible. It is very similar to the care you would get at the clinic of a major US city public hospital.
