Cafe Rojo serves quite a few vegetarian dishes. I usually just go there for a coffee and dessert. Its located on ave.7 between calle 3 y 5 west of Bar Poas.

There is a restaurant called Vishnu. They have one or two locations in San Jose. Several of my friends like it.

Look up Vishnu Costa Rica on Google for directions and reviews.

check out las mananitas, a mexican restaurant. You get enchilladas, fajitas, quesadillas, and tortilla soup all of which are vegetarian.

Also, for the record there's a great restaurant on the corner of Avenida 2 and calle 15. Once you tell them you're a vegetarian, they have a couple good suggestions. Also, breakfast is really good there.
